Your first visit
If you are here as a new patient to our general family practice, your first visit will be a comprehensive new patient examination. If you are referred here for a consultation for implant or other treatment, your first visit will be for a specific consultation. Either way, at your initial appointment we will interview and examine you, and gather whatever other records and tests we require to thoroughly diagnose your condition. We will then review your case with you including options and recommendations, and give you a chance to ask any questions. Occasionally, urgent treatment can be performed on the same day as the new patient examination or consultation.
Remember to come a few minutes early to fill out a new patient registration form; alternatively you can find the form under Patient Forms here on the website and fill it out in advance from home and submit it online.
Before your first appointment
In order to provide you with the best possible care, please bring the following with you to your first appointment:
- A list of your current medications;
- Any current radiographs (“x-rays”). If you call your former or referring dentist in advance of your appointment, they can often forward any recent radiographs to us and reduce the number that need to be taken in our facility;
- If you have medical or dental insurance, please bring your coverage information, such as plan and ID numbers. Our offices are equipped to send most claims electronically (paperless), so you do not need to bring insurance forms.
Note: A parent or guardian must accompany all patients under the age of 18 to the consultation visit.
In answer to one other common question, while most of our implant patients are referred by their regular dentist, you do not need a referral to see us for consultation or a second opinion.
